Note publique d'information : La compliance thérapeutique est un problème de santé publique. Nous avons tenté d’identifier
une corrélation entre les états anxieux ou dépressifs des patients et leur compliance
à un projet de soins. Nous avons utilisé le retour en consultation comme variable
de compliance. Les dossiers de 146 patients ayant remplis le questionnaire de Hamilton
ont été sélectionnés. Les résultats ont été introduits dans la base de donnés du service
hospitalier. Nous avons distingués la population en deux groupes : les patients revenus
et les non revenus dans les 5 ans suivant la primo consultation. Nous avons ensuite
pratiqué une analyse univarié, puis multivarié afin de déterminer quelles variables
étaient significativement différentes (P<0,05) entre les groupes. Le premier résultat
constaté était que seulement 43% avaient reconsultés dans les 5 ans. Nous avons identifiés
3 variables significativement différentes entre les 2 groupes: Le sexe du patient
(p=0,0184), le score d’anxiété (p=0,0162) et la présence d’un traitement par statine
(p=0,0493). La variable score de dépression n'était pas significativement différente
entre les 2 groupes. La variable retour en consultation s’est avéré discutable pour
mesurer la compliance compte tenu de l’absence d’articles l’utilisant dans la littérature.
Cependant, le faible taux de patients reconsultant motive de rechercher la signification
de cette variable. Concernant l’anxiété et la dépression, seul le score d’anxiété
a pu être corrélé au retour en consultation, sans présager pour autant de son influence
dans la compliance aux soins. Le sexe du patient et la présence d’un traitement par
statine étaient aussi corrélés au retour en consultation.
Note publique d'information : The therapeutic complicance is a problem of public health. We tried to identify a
correlation between the anxious or depressive states of the patients and their compliance
with a project of care. We used the return in consultation as variable of compliance.
The files of 146 patients have filled in the Hamilton questionnay where selected.
The results of the scores of anxiety and depression where introduced into the clinical-biological
basis of the hospital department. We distinguished the population in two groups: the
returned patients and the patients not returned in 5 years according to the first
consultation. We practised an univariate analysis followed by a multivariate analysis
to determine wich variable where significantly different (p<0,05) between the groups.
The first important result was that only 43% of the patients came back within 5 years.
We identified 3 variables presenting significant differences between 2 groups: the
sex of the patient (p=0,0184), the score of anxiety (p=0,0162) and the presence of
a treatment by statin (p=0,0493). The variable “score of depression” did not present
a significant difference between the 2 groups. The variable return in consultation
turned out to be debatable to measure the therapeutic compliance considering the absence
of articles using this variable in the literature. However, the low rate of patients
that return on consultation motivates to look for the meaning of this variable. Concerning
the anxiety and the depression, only the score of anxiety was able to be correlated
on the variable to return on consultation. The sex of the patient and a treatment
by statin where also correlated with the return in consultation.
