Note publique d'information : La rencontre avec autrui est au coeur de l’expérience humaine. En psychiatrie elle
doit répondre à certains critères pour avoir réellement lieu et permettre la mise
en place du processus thérapeutique. Quels éléments permettent de rendre la rencontre
thérapeutique et d’envisager l’autre dans sa globalité ? Nous centrons ce travail
autours du cas d’une patiente schizophrène rencontrée à Mayotte. En première partie,
nous décrivons cette situation clinique. Nous exposons ensuite les bases de la réflexion
ethno psychiatrique et des exemples d’expériences hospitalières Africaines. Dans un
deuxième temps, nous nous intéressons aux caractéristiques de la rencontre dans le
domaine de la psychiatrie. Nous évoquons des concepts tels que l’ambiance, la théorie
de l’esprit, l’empathie. Puis nous abordons le champ de la schizophrénie. Ce diagnostic
perturbe les éléments de la rencontre. Comment la rendre malgré tout possible et thérapeutique
? Nous réfléchirons d’abord aux outils à mettre en place en relation duelle patient/thérapeute
puis au travers de l’institution. Ainsi nous verrons l’importance de l’alliance thérapeutique
mais aussi d’une institution « désaliénée ». Dans chacun des axes de réflexion cités,
nous avons développé différents regards : phénoménologique, psychanalytique, institutionnel,
neurobiologique. En dernière partie, nous revenons à la situation clinique. Cette
situation de trans-culturalité a permis de mettre en avant des caractéristiques universelles
de la rencontre avec un patient. Nous concluons par les éléments qui doivent favoriser,
en pratique psychiatrique quotidienne, l’approche de l’altérité du patient de manière
authentique et donc soignante.
Note publique d'information : Meeting with other people is at the center of human experience. In psychiatry meeting
has to answer some criteria in order to really exist and to facilitate the therapeutic
process establishment. Which elements make it therapeutic and allow a global approach
of other people ? We organize this work around a schizophrenic woman case study met
in Mayotte. In the first part, we describe this case study. Then we exhibit basic
principles ofethnopschiatry and some models ofhospital experiments in Africa. In a
second stage, we are interested in the meeting particularities in the field of psychiatry.
We mention concepts lilce atmosphere, theory ofmind, empathy. Then we talk about schizophrenia.
This diagnosis disturb meeting components. In spite of this, how make the encounter
possible and therapeutic ? On one hand, we think about therapeutic strategies in dual
(patient/therapist) relationship and on the other hand we think about institutional
strategies. Thank to this, we underline the importance of therapeutic alliance and
also of the hospital desalienation. In each topic exhibited, we have developed different
viewpoints : phenomenological, psychoanalytical, institutional, neurobiological. In
the last part, we return to the case study. This transcultural situation allow us
to emphasize universal characteristics of the meeting with a patient. We finish by
listing elements who facilitate, in daily psychiatric practice, an authentic and then
therapeutic approach of the patient otherness.
