Note publique d'information : Introduction: Amyloidosis is a disease caused by infiltration of tissues by an amyloid
protein. The most frequent cardiac amyloidosis are light chain (AL) amyloidosis, hereditary
transthyretin (TTR-h) and wild transthyretin (w-TTR). Objective: The objective of
this study was to investigate the recruitment of patients with cardiac amyloidosis,
their baseline characteristics, their prognosis and to determine early prognostic
factors. METHODS: From late 2009 to July 2016, we conducted a cohort study of patients
referred to the expert center of the Henri Mondor Hospital (Créteil, France) for suspected
amyloidosis. We included patients with AL amyloidosis, TTR-h and w-TTR. We noticed
for all patients demographic, clinical, laboratory and ultrasound characteristics,
as well as the occurrence of major events (death, organ transplantation or assistance).
Results: Of the 942 patients sent to our center, 884 patients were included, of which
162 AL amyloidosis, 203 h-TTR and 141 w-TTR. Over 6 years, recruitment has grown exponentially.
Mostly neurological at the beginning, it quickly diversified (mainly cardiological
then hematological, nephrological ...). The population we studied consisted of 60%
men, the median age was 69 years. Age was significantly higher (83 [79; 87], p <0.001)
and there were more men (86%, p <0.001) in the w-TTR amyloidosis group. Cardiac failure
with NYHA III-IV dyspnea was higher in AL amyloidosis (60%) and w-TTR (54%, p <0.001),
while neurological symptoms were predominant in h-TTR (62% p <0.001). The elevation
of creatinine, NT-proBNP and troponin was greater in AL amyloidosis and w-TTR (p <0.001),
hemoglobin and sodium were lower in AL amyloidosis. Interventricular septum was thicker,
LVEF and global strain were lower and E/A and E/Ea ratios were higher in w-TTR amyloidosis
(p < 0.001). The survival without major event was 59% for AL amyloidosis, 79% for
w-TTR, 81% for h-TTR against 88% for patients without amyloidosis (p <0.001).
